Grant making:as a funder, the health trust invests in community-based organizations and public agencies advancing health equity and systems-level change.annually, the health trust commits to invest over $2.3 million through its endowment, which provides a direct benefit to residents of santa clara and northern san benito counties. These restricted dollars are awarded through health partnership grants, where the health trust invests in organizations, programs, and projects that seek to address health inequality, prioritizing deeply impacted communities. The health trust focuses on key drivers of health, like (continued on schedule o) food and nutrition, chronic disease prevention and management, housing support, and other approaches to building health equity.the health trust's philanthropic investments are guided by a regional health-equity framework that seeks to align government, philanthropy, and the community around shared priorities. Funding approaches include multi-year health partnership grants, community grants, and rapid response grants that collectively strengthen the capacity of local organizations to drive measurable and lasting impact.during the year ended june 30, 2025, the health trust awarded $3.65 million, partnering with 77 nonprofit and public-sector organizations through grants, convenings, and collaborative initiatives. Grantmaking supported a range of strategies from rapid-response efforts addressing urgent community needs to long-term investments in food-system transformation, housing stability, and policy advocacy that tackles structural inequities.in an effort to deepen its impact, the health trust remains dedicated to collaboratively funding initiatives with like-minded partners.
