ST. Joseph's health at home ceased operations in october 2023 and is in the process of winding down. ST. Joseph's health at home was the only hospital-based certified home health care agency in onondaga county. Services included skilled nursing, home health aides, physical, speech, and occupational therapy, social work visits, needs assessment and care plan development, and patient education related to current medical conditions and chronic illness.
