Is a physician group formed in 2020 as an innovative response to the need to provide hospital-level care in the comfort of a patient's home during the covid-19 pandemic. Promedica physicians at home, INC. Will combine promedica's extensive network of in-home care providers with effective care at home for patients with select chronic conditions or diagnoses such as heart failure, pneumonia or COPD. The care team consists of physicians, nurse practitioners, registered nurses, and may include other healthcare professionals, such as physical or occupational therapists, as needed. Additionally, promedica acute care at home expanded its services to include care for select oncology patients by collaborating the promedica cancer center.
