Health access program: health access program: a national program that removes barriers to healthcare for uninsured and low-income individuals in the united states by delivering solutions through their healthcare providers. The health access program provides solutions to healthcare workers at clinics serving uninsured and low-income populations. Program services include transportation, screening and assessment support, and connections to follow-up care. Support for healthcare workers includes technical training, implementation assistance, and data collection and reporting. The program consisted of the following initiatives: rides for health equity: to address transportation barriers for low-income patients, sostento enabled free and charitable clinics to order rides through the uber rideshare platform. In 2024, sostento provided 8,789 rides to 2,419 unique patients, 94% of whom lived below the poverty line. Behavioral and mental healthcare: to improve access for uninsured and low-income individuals, sostento completed an assessment, design, and solution development process resulting in a continuum of care model that integrates transportation, screenings, and connections to care. Pilots were developed with clinical partners in atlanta and chicago, and resources were mobilized for launch lanning in each city. Maternal and child healthcare: to support new and expectant mothers in the mississippi delta at high risk of poor health outcomes, sostento participated in a cohort to develop solutions for regional care centers. Sostento led the development of the transportation and data collection components of the project.
