Trillium place, through a collaborative process, has reduced overall behavioral health emergency department visits to carle health methodist hospital by 11% in 2024, and has reduced behavioral health emergency department visits by 43% in the same period. Trillium place also measures how effective their services are in transitioning individuals who have been hospitalized back into the community for follow-up care and saw a 16% increase in the number of referrals (for eligible patients) that were made from hospital inpatient units for community-based services prior to the patient being discharged.
