The cohqa grant program:the grant program is funded through the surplus generated by a partner organization, the central oregon independent practice association's (coipa) oregon health plan contract with pacificsource. The funds are offered at up to $40,000 per year, for projects adsressing the following priorities: 1) provider led health care innovation: goal to implement intervention(s) specific to health care delivery systems, clinicians, and/or patients that focus on the quadruple aim of: improved quality, reduced cost, improved patient experience, and improved provider and staff experience in the communities that cohqa serves.2) community led healthcare innovation: innovations to improve comunity health, health care, and patient or provider experience can and should come from the community itself. We encourage community members, as consumers of healthcare, to partner with healthcare providers to develope and implement fresh ideas to improve care delivery.3) vulnerable or underserved populations: cohqa would like to improve health care for the most vulnerable or underserved members o four community. We support projects that address disparities in care due to socioeconomic, racial, or special needs (chronic disease, disabilities, etc)4) clinical integration and multisector coordination: implementation of intervention(s) that advance clinical and financial coordination and integration of stakeholders in the communities we serve.5) skill transfer: efficiently and effectively transfer skills and learned processes to other providers, clinics, or community organizations in a colloborative fashion.

Cohqa grant funded projects:cohqa will seek to apply for grants to fund larger, multi-partner projects in the community, and and if awarded, administer and perform the specified activities to successfully meet the goals of the awared grants. Cohqa will specifically target grant opportunities that speak to our unique strengths as a bridge across many disparate actors in the health care community in central oregon. This will allow us to develope, implement, and evaluate innovative new health care delivery systems and transformative quality improvment ideas that require coordination across providers, health organizations, and communities to successfully implement. Particular focus is on 1) improving coordination of care across patients' entire care team, 2) increase patient knowledge of, and engagement in, their treatment and health care options, and 3) exploring alternative payment models that can better align incentives among patients, providers, and insurers.
