The pathways community hub institute (PCHI) supports communities to improve health and transform their local healthcare delivery systems by implementing the pathways community hub institute model. The PCHI model of community-based, outcome-driven care coordination is designed to improve health and quality of life for those at greatest risk for poor health outcomes. PCHI accomplishes its mission through PCHI model development, research, advocacy, technical assistance, education, training, and certification. PCHI assures fidelity to the PCHI model, sets its standards, and is the certifying body for: pathways community hubs, pathways agencies, technology vendors, and community health worker instructors.
