Grants were awarded for the following: diabetes education and prevention; prescription medication program assistance; program for ages 6 - 14 to address childhood obesity with physical activity, healthy eating and self- esteem; implementation of the triple aim initiative including 80 organizations; smoking cessation program and tobacco intervention; crisis assessment, referral and engagement team available 24 hours each day that can respond to behavioral health crises; initiate a care team approach for mental health; mobile dental clinic in an area without dental care; dental consortium to build capacity in the community including evening hours for adult dental services, including an annual mega clinic to provide free of charge portable dental clinic; school system wide health program to help decrease obesity rate in marion county; assist to increase the latino community's diabetes prevention and provide linkage to treatment
