Chesapeake-Potomac Home Health Agency strives to increase physical and physiological functioning, and spiritual well-being of patients through home health services. During FY24 Chesapeake-Potomac Home Health Agency admitted over 1,817 patients to care and provided over 26,287 visits to patients residing in Calvert, Charles and St. Mary's Counties. CPHHA specializes in the care of geriatric patients with over 81% of our patients being over 60 years of age. Our average daily active census is 210 patients.

Chesapeake-Potomac Home Health Agency supports its non-profit members:three local community hospitals providing healthcare to Chesapeake-Potomac Home Health Agency's community, CalvertHealth Medical Center, MedStar St. Mary's Hospital and University of Maryland Charles Regional Medical Center (via The Chesapeake Potomac Healthcare Alliance, LLC), as well as the St. Mary's County Health Department through periodic distributions of grants to those entities to support the exempt purpose of such entities.
