Johns hopkins home care connectionsjohns hopkins home care connections was created in 2014 to manage the home based need of ehp family business for patients outside the normal pharmaquip and home health service areas for pediatrics and adult populations. Over the last two years connections has grown to accommodate all lines across johns hopkins healthcare (JHHC) including ehp, priority partners, usfhp, and medicare advantage. Connections operates as a third party administrator, linking patients outside the pharmaquip and home health service areas to provide throughout maryland and it's surrounding states. The scope of services include skilled nursing, physical therapy, occupational therapy, speech therapy, medical social services, durable medical equipment, and infusion therapy. These services are provided through our network of vendors that have met the criteria of both JHHC, pharmaquip, and home health services to provide care to our patients following the johns hopkins standards of excellence.
