Bridges to health (b2h) is the columbia gorge regional implementation of pathways. Pathways is a centralized mechanism that coordinates, tracks and measures both the process and the resources that enables distributed community care coordination of those served. Pathways ties payments to milestones that improve client's health and well-being. Regional approach to a community health assessment (cha) and community health improvement plan (CHIP) in partnership with 4 hospitals, 4 health departments, a federally qualified health center, county mental health agency, early learning hub, united way and pacificsource community solutions. The regional cha and CHIP has led to a higher quality product and increased collaborative approach on priority areas for improvement.
