As a federally qualified health center (FQHC) serving kansas city and the surrounding bi-state community, swope health services (swope health) served 54,716 unique patients and patients received 338,108 services in 2024. Accredited by the joint commission and the commission on accreditation of rehabilitation facilities (carf), swope health operates a full-service clinic in kansas city, missouri, plus missouri satellite clinics in belton, independence, hickman mills, northland and maple woods, two clinics in wyandotte county and one clinic in leavenworth county, kansas. Swope health operates three school-based satellite clinics at emmanuel family and child center, operation breakthrough, and the excelsior springs early learning center, and operates mobile medical and dental units which visit schools, shelters and other outreach sites. The mobile dental unit delivers oral health prevention services to 82 locations. Close to half of swope health's patients are african american, one-third are white and most of the remaining patients identify as more than one race or hispanic/latinx or american indian/alaska native. We are experiencing growth in our hispanic/latinx patient populations at most locations, consistent with recent national census data. We have achieved primary care medical home (PCMH) accreditation from the national committee for quality assurance (ncqa). In 2024, swope health was recognized at the gold level for the clinical quality award from the missouri primary care association; this recognition means that we met or exceeded a goal of 80% on mpca's selected CMS measures for healthcare effectiveness data and information (hedis) and uniform data system (uds).in 2024, swope health enrolled the first participants at the pace KC adult wellness center. Participant enrollment grew throughout the year. In 2024, swope health also broke ground on a behavioral health and intellectual/developmental disability (bhidd) residential location, called 87th street place. In addition, swope health is partnering with the kansas city kansas community college and community america credit union for the kansas city kansas community education, health and wellness center which will include a women's and children's center offering medical, dental, and behavioral health services operated by swope health. Swope health behavioral health provided services to 7,075 individuals in 2024. Those individuals received more than 135,000 visits, some which occurred on the same day. These services include community support, individual and group therapy, medication assessment and management, substance use treatment, residential treatment, crisis intervention, and case management. These services are available for ages three and over. Adults are also offered psychosocial rehabilitation, integrated treatment for substance use, opioid treatment, and employment services. Swope health was an integral partner in the rollout of missouri's 988 emergency crisis line providing staffing, mobile response, and education and outreach. Swope health's behavioral health, medical and dental services teams continue to improve our service integration. This has resulted in increased care coordination, resource attainment, and better patient outcomes. In 2024, we continued to provide services in multiple ways in order to improve access to care. This includes telehealth with remote providers and the ability for individuals to receive care in their homes using video and/or telephone. The majority of services are now back to face-to-face, but we continue to offer multiple ways to receive care based on the preferences of the individuals served. Access to care remains a top priority as we look toward new models of service delivery, combining remote and on-site services to meet the needs of clients. In 2024, swope health assisted 3,339 unhoused patients with health care, housing and other essential services. Of these unhoused patients, 93% of them were at or below 200% of the federal poverty level (FPL). Outreach services provided 2,900 homeless intake screenings. Swope health staff members are active in the homeless community and often visit shelters, community day centers, and homeless encampments to assess basic medical and behavioral needs of the unhoused.
