Fairview Physician Associates Network (FPA) currently is accountable for providing a broad array of healthcare services for over 525,000 individuals through our contracts with purchasers and payers. The network assumes risk for over 438,000 member lives in shared savings incentives, including 120,000 member lives in defined network products. Although many of FPA's health improvement and population management initiatives have been developed to increase quality, improve the patient experience, and decrease cost to the individuals served through our shared savings care system contracts, many of the benefits accrue to the entire population served through FPA affiliated clinics and Fairview hospitals (clinical integration). The organization provided $243,370 in grants to various health care organizations (see Schedule I). These funds were used to distribute 1.2 million meals through 20 food pantries and mobile food banks. Approximately 15,000 families were served monthly during 2024. Another use of the funds as for the Learning to Lead program offered by C-Suite Resources which is designed for physicians to cultivate their leadership skills and promote professional growth. Finally, grants funds were used to implement and integrate eCW Behavioral Health Care Management modules to organize patient and care team information in a centralized location, enhance data analytic and reporting capabilities, reduce staff time in managing registries and provide organizational tools for patient management.
