Trinity home health services (THHS), located in livonia, mi, manages and supports several home health care and hospice sites affiliated with trinity health. In addition, THHS operates home care/hospice sites serving residents in california, connecticut, delaware, florida, illinois, indiana, iowa, maryland, massachusetts, michigan, nebraska, ohio, and south dakota. THHS responds to individual and community health needs by providing health services to individuals in their homes. Services include skilled nursing, home health aides, therapy, social work visits, needs assessment and care plan development, and patient education related to current medical conditions and chronic illness. Hospice helps people with life-limiting illnesses live their remaining lives alert and pain free, nourished by the support of their loved ones.
