The last patient of henry ford allegiance specialty hospital (hfash) was discharged in april 2020. Its medicare contract with the CMS was voluntarily terminated in march 2021 effectively closing long-term acute care hospital operations at the facility. The hfash governing board formally authorized the permanent closure of hfash operations in july 2021. The michigan department of licensing and regulatory affairs approved the hfash plan of closure effective september 30, 2021. The acute care hospital license was voluntarily surrendered.a certificate of dissolution was filed april 30,2025 to disolve carelink of jackson.
