Housing support services - unity house offers housing support, case management, care coordination and nursing services options for adults living with mental illness, HIV/AIDS and/or chronic medical conditions. Services offered support individuals to live safely and self sufficiently in the community with dignity and independence. Services and programs include transitional community residences and apartment services, a situational crisis bed, supportive housing, empire state supportive housing initiative services (esshi), community housing assistance program, case management, health home care coordination, medical transportation and nursing services. Served 33 in community residences, 83 in transitional apartment services, 3 individuals in a situational crisis bed, 110 were served in omh supported housing program or housing programs for people living with HIV/AIDS, 49 in esshi, 60 served in community housing assistance program, 122 individuals received health home care coordination, 33 were provided case management services and 327 served by nursing services.

Domestic violence domestic violence services provides crisis intervention, a confidential emergency forty bed shelter for adults and children, housing support services, counseling, social service advocacy and accompaniment, children's services, parenting and life skills education, information and referral, legal assistance/representation and emergency medical care. 186 adults and 96 children found refuge in forty bed shelter, 1,945 individuals received non-residential services, 503 adults were provided legal and support services, 545 individuals provided transitional and long-term rental assistance and support, received 1,615 calls from the rensselaer county domestic violence hotline, 255 survivors participated in domestic violence support groups and therapy services. Uhdvs staff held 54 community educational and awareness events.

Community resources the front door - the community resources program provides front line support for rensselaer county residents and surrounding communities, offering crisis intervention, legal and social services advocacy, emergency food, clothing and housing assistance, and holiday food baskets and gifts. 207,288 meals distributed from the food pantry, 739 pre-screenings, and 696 households helped with SNAP benefits through our nutrition outreach and education program.

Northeast career planning (NCP) empowers individuals with disabilities or other barriers to achieve their goals toward self-sufficiency and personal success. NCP offers a wide range of programs including vocational evaluation, vocational training, and job placement and educational services. Our team served 711 individuals and approximately 25,750 units across eight counties in upstate new york during 2024.

Food services-plays a vital role in promoting wellness and food security across our programs and the broader community. With a focus on dignity, nutrition, and consistency, our team ensures that individuals of all ages have accesss to balanced, nourshing meals every day. Over 150,000 nutritious meals prepared and delivered to children enrolled at a child's place. Over 43,000 meals served to residents of sojourner place, our domestic violence shelter. 58,878 nutritious meals served through our vended meals program to children at local daycare centers. 26,964 lunches served through our community meal program. Thousands of meals provided through partnerships with shelters, supportive housing programs and other local agencies.
