Clinical programs: the purpose of n3cn clinical programs are to provide high value services to the state of north carolina to improve the health of medicaid beneficiaries and improve quality in the healthcare delivery system. Some of the core initiatives include, but are not limited to, the following: complex case management, care coordination, foster child program, pharmacy program, and quality improvement program for advanced medical homes. The program develops and implements population management initiatives that are supported by data analytics and population health tools. Using analytics, the program targets individuals that would benefit most from care management services to high need medicaid enrollees. N3cn also promotes quality of care and health equity in the primary care practices that care for medicaid enrollees.

Data analytics: performance measures and data analytics have been established on a statewide basis. The process includes developing systems for collection and analyzing data and reporting performance measure results. These reports are created at the program, regional, and practice level, and provide the basis for measuring improvement and impact on the enrolled population.
