Uhf awarded $240,000 in grants in fy25 to support ongoing programs to address polypharmacy in nursing homes, to support clinical quality improvement training, and to support research on individual experience and outcomes for maintaining coverage during the unwind of the continuous coverage requirement process. Uhf hosted the 2024 medicaid conference, which brought together over 850 policymakers, providers, insurers, community organizations, and policy experts statewide to share and discuss important issues in new york's medicaid program. The conference, shaping the future of care integration" featured a keynote by amir bassiri, the state's medicaid director. Other sessions included panels on coordinating perinatal care, integration to provide whole-person care, and the future of integrating social needs into medicaid.other program expenses included general operating expenses not allocated across programs and two new projects. The health equity fellowship launched in fy25 with the selection of 8 fellows who embarked on the start of an 18-month experiential learning opportunity blending collaborative leadership development, policy, and systems change thinking. Fellows partner with a community-based organization (cbo) and collaborate on a project to address a specific health equity need within their served community. The new york state 1115 waiver project builds on uhf's historic medicaid institute work with a new focus on integrating physical, behavioral, and social care to improve population health.
